How can I make additional changes to my submitted forms?
If you have left a form incomplete, you can continue filling out your form by selecting the ANSWER button (in mobile or desktop). If you want to make changes to a previously submitted form, you can select EDIT form.

After making changes you will have to PREVIEW and SIGN/SUBMIT your form for the provider to receive it.
How can I view my forms after I have submitted them, without doing edits?
You can easily view your forms after you submit them from the VIEW button next to the form's name.

To see previously submitted forms, select the HISTORY (Available after you have submitted a form 2+ times).

Why was logged out of UpBup?
For security reasons, UpBup will log you out after 15 min of inactivity.
How can I add contacts to my emergency contact or pick up list?
