If your care provider requires you to fill out a Income Eligibility Form for the Food Service Program, it is important that all necessary information is included. In this article we cover the information needed by the state to consider this form "Complete" and how to ensure that you include all of this information when submitting it in UpBup. 


1. Log in app.upbup.com with your previously created credentials. If you need to recover your password find instructions here


2. Select ANSWER the Income Eligibility Form for the Food Service Program



3. You can collapse the different sections to view the Steps we will be referring to in this article 


4. The Applied Child Information will be filled out from your other forms


5. You MUST add the child mentioned in the "Applied Child Information" in PART 1 section. In addition to any other child/children attending camp





Input the information for that child 



If there are more children, repeat the steps above for Child #2 after filling the information for Child #1




6PART 3 Section: You MUST add the children included in PART 1, IN ADDITION to yourself, and any adult in the household and their respective income information







7. Fill out PART 4 and PART 5



9. Preview your form. Please note that any children included in PART 1, will also have to be added to PART 3



10. Sign and Submit your form
