Some providers may require a fee for you to submit a form. Here are the steps you will need to follow to ensure your form and payment get to your care provider:
1. Log in app.upbup.com
If you forgot your password you can always reset it by following these instructions Login and Password Reset
2. Find your provider's Form Dashboard
3. Find the form you will need to submit and pay. Please note that your provider may have more than one form that requires payment, please ensure that the form name matches with the form and payment your provider requested.
You can always filter on only requested forms using the filters at the top of your dashboard
4. Click ANSWER next to the form's name
5. Answer all required questions and upload required documents if needed
You can upload JPEG, PNG and PDF files from your desktop files, or by taking a picture of the file with your mobile
6. Once all required questions are filled out, and all required documents are submitted, you will be able to click the blinking PREVIEW button at the bottom right of the screen
7. Review the PDF of your complete form
8. Click PAY FORM FEE
9. You can confirm the form fee you are paying for, and input your credit card information. Then click PAY NOW
10. If your payment is successful you will be directed back to the questionnaire
11. Please click PREVIEW again
12. Click SIGN AND SUBMIT
13. Sign and Submit your form
14. You will be able to review your form and payment details from the Forms Dashboard
