If your care provider requires you to fill out a Meal Benefit Income Eligibility Form, it is important that all necessary information is included. In this article we cover the information needed by the state to consider this form "Complete" and how to ensure that you include all of this information when submitting it in UpBup.
1. Log in app.upbup.com with your previously created credentials. If you need to recover your password find instructions here
2. Select ANSWER the Meal Benefit Income Eligibility Form
3. You can collapse the different sections to view the Steps we will be referring to in this article
4. Fill out the child information section, this is required
5. You will need to list ALL of the household members who are children in STEP 1 (this includes the child that you included in the Child Information section). Select ADD
Input the information for that child and any other children that may live in the household
If there are more children, repeat the steps above for Child #2 after filling the information for Child #1
6. STEP 2 section:
If you DO participate from SNAP, TANF, or FDPIR please select YES, provide your Case number and skip to STEP 4.
If you DO NOT participate from SNAP, TANF, or FDPIR please select NO, and proceed to fill out STEP 3.
7. STEP 3 Section:
Answer the highlighted questions, and ADD any income for ALL adult household members by selecting ADD
It is REQUIRED that all household members fill out the income details, if there is no income from a detailed section, please input "0", if there IS an income, please indicate the frequency
8. STEP 4 Section: Fill out every field in this section with your contact information
9. Preview, Sign and Submit your form
REMEMBER: STEP 1 and 4 should NEVER be blank, and Step 3 can ONLY be blank if you have a Case number
