To submit your eligibility applications to the hub you will need to follow these steps:

1. Select "Create New Application" - Review the questions presented in the short application and confirm you are eligible to apply. If all the answers are "Yes" you will be able to move forward. 


Please note that the 'Login for Registered UpBup account' will only log you into your account, if you are adding a new application you will need to select "Create new application" even if you already have an account. 



2. Select the new type of application that applies to your case, if the hub you are applying to has different applications. Please note some hubs won't have this dropdown as they only offer one type of application. 


3. Fill out the required fields and click SUBMIT. You will see messages indicating that an invite was sent to your email. That email will contain a link that will allow you to create a password and activate your account, or login to your existing account. 


If you have an existing UpBup account, please use the same email, this will allow you to use your existing account to submit and review your application. 




If you want to submit an application for multiple children, proceed to change the child's name and DOB within the short application and click SUBMIT again. You should use the same parent email for both children.


4. Follow the link in your email and create a password to log in. 



5. You will be directed to your application form. You can filter on "Missing Required Questions" if needed. Proceed to fill out the form. 




5.1 You will reach the Preferences section, which allows you to indicate which provider(s) would you prefer. You will be able to select a 1st, 2nd, and 3rd choice. 

You can view the location of all providers in reference to your address by selecting SHOW LOCATION 

 




You can view the route from a specific center to your address by selecting the DIRECTION button next to the center's name.



You can toggle this view from Transit, Driving or Walking distance



6. Depending on your type of application, you may have to upload certain supporting documents. You can select UPLOAD and select the appropriate files from your computer. When filling out the form on mobile you will be able to use your phone's camera for this as well. 





7. Once the all required questions in the application are complete you will see the PREVIEW button blinking 



Review the form


8. Select the blinking SIGN AND SUBMIT



9. You will be redirected to the Forms Dashboard, which allows you to confirm your application and documents have been submitted
